Community Use of Facilities

Application

Applications can be obtained at the Security Office located at District Office/Samoset MS - 51 School Street, Lake Ronkonkoma. These applications are available during school hours. (A sample application is available by clicking here. Please note that this online application is NOT a substitute for the one available at the office listed above). All applications must be received a minimum of four (4) weeks prior to the date requested. All applications must be completed in its entirety (names, dates, times, locations, phone numbers, fees, etc…). The Use of Facilities Application must be signed by the Director in order for approval. Simply submitting an application does not grant approval for usage. All Use of Facilities Applications for must include the player roster that includes names and addresses of all participants.
